グループ定期をしており、債権請求フォームを送ろうと四苦八苦しているものです。
わからないことだらけで、、、
こちらで質問させていただきたいのですが、
a Details of your original investment including dates any amounts were paid, amounts that were paid, company bank details of any amounts paid into, bank details from which any amounts were paid
→もともと預けたのは２０年近くも前の事で現金を持ち込んだのか日本から送金したものを預けたのか、記憶もなく、証明できるものも残っておりません。毎年更新してきて預け始めた当初の金額もわかりません。
最新のステートメントの元金を記載しておけばよいのでしょうか？
b A summary of any subsequent payments received by way of dividend/interest paid, dates paid, amounts paid, bank account details which received the payments, Company bank account where amounts were paid from.
→解約していないため更新のたびに元金に上乗せされておりました。この場合は何を記載すればよいのでしょうか？
c Copies of any paperwork in relation to your investment and reinvestment as appropriate, any contracts with the companies.
→数年分のステートメントは残っておりますが、最新のものだけ添付すればよいのでしょうか？

宛名はStephanie Jeffreys さんでしょうか？
それとも管財人であるTimothy Wilson Downesさん、Russell Malcolm Mooreさん宛てでしょうか？
